Parent & Player Feedback Forms for the 2022-2023 Lynx Season are now available to complete on the Lynx website.
As the season draws to a close, this is a time to reflect on your experiences and provide feedback & suggestions to the LGHA. Your valued input helps us to grow & improve as an association, while also ensuring that we are meeting the needs of our players and their families.
- Separate Player & Parent links - Each form asks different questions. Please let your player complete their form with their own words, thoughts, & ideas.
- Anonymous Forms - Each player/family will be issued a code which may be used multiple times (player, parent, step parent, guardian, etc). The codes are unique & randomly assigned. Submissions will only be accepted if the code matches one from the set given to the team. Team staff do not have access to any survey submissions. We encourage your feedback and realize that anonymity helps provide the safety to share. These codes are non-identifiable.
