Please use this online form to request a signed Permission To Tryout form for Tryouts that can take place beginning April 17, 2024. This form can be used to gain permission from the LGHA to tryout for another Association at a level above what the LGHA has to offer at a particular age group.
The LGHA does not entertain lateral player movement (player releases) at this time. (ie. Lynx BB to Ice Kats BB)
Once submitted, LGHA requires up to 72hrs to process a PTT Form Request.
This is NOT a Player Release
This form must be used when a player wishes to attend a tryout hosted by another OWHA association. A player must receive a signed Permission to Tryout from their last registered OWHA association in order to be eligible.
Signed Permission to Tryout forms are valid as of 12:01 am on April 17th, 2024 to May 31st, 2024.
