LGHA REFEREE REQUEST FORM

Team scheduled HOME GAMES must be submitted to the LGHA in order to notify Referees, Ice Scheduling, & Website Admin.

Complete this form to submit your team scheduled HOME GAMES.
Please fill out a new form for each HOME GAME scheduled or Upload a List (Recommended following the Scheduling Meeting)

Do Not complete this form for AWAY or TOURNAMENT GAMES.
Please submit all AWAY & TOURNAMENT GAMES for the Lynx Website Calendar here: https://lgha.net/Forms/1911/

Maximum # Files: 10. Maximum File Size: 4MB.

Please ensure that each & all home games listed have the following complete info: GAME DATE & TIME, ARENA, OPPONENT, & GAME TYPE (Regular Season, Exhibition, Playoff, or Playdown)
