Complete this form to notify LGHA of a Cancellation of a Game or Practice
IMPORTANT NOTE: When Cancelling Regular Season & Playoff/Playdown Games, teams MUST follow the OWHA Cancellation Procedures
(Detailed in the 2023-2024 OWHA Rules & Regulations Handbook - Page 7, Section 10 "Cancellation and Rescheduling Rules")
This form does NOT notify an opposing team of a cancellation!
This form will only notify LGHA and our local Referees when the cancellation submitted is a Home game.
For all game cancellations, you must still notify all parties first, then LGHA by completing this form (follow the above OWHA Cancellation Policy)
